Un trasplante de células madre restaura la vista a ratones ciegos -Científicos gallegos aseguran que se trata de un importante avance hacia la medicina personalizada

Investigadores del Centro de Biología del Desarrollo RIKEN (Japón) han conseguido restaurar la visión en ratones ciegos con degeneración en la retina en fase terminal, una de las principales causas de pérdida irreversible de visión en adultos, gracias a un trasplante de células madre. Este avance, que publica la revista «Stem Cell Reports», puede siginificar el primer paso para revertir en un futuro la ceguera por distrofias en la retina.

«Por primera vez, este estudio consigue, usando los dos tipos de células madre pluripotentes inducidas, embrionarias y derivadas de células somáticas, que arraiguen en la retina y además, que sean funcionales, es decir, que los ratones puedan volver a detectar luz. Esto es un avance conceptual, aunque ya había estudios precedentes que habían dicho lo mismo, aunque hasta ahora no se había conseguido que se anclasen en la retina y que fueran funcionales», explica el investigador gallego Miguel Fidalgo, responsable del Grupo Stem Cells & Human Diseases (Células Madre y Enfermedades Humanas) del Centro Singular de Investigación en Medicina Molecular y Enfermedades Crónicas de la Universidad de Santiago (CiMUS).

Ya en 2014, un equipo internacional de investigadores probó la eficacia de implantes de células madre para curar la ceguera a pacientes con enfermedades de retina. Ahora, la técnica desarrollada por los investigadores japoneses, basada también en el trasplante de células madre, logra restaurar la visión en ratones ciegos con degeneración retiniana terminal.

El estudio demuestra que, tras el trasplante de células madre, el tejido retinal derivado de las células madre pluripotentes inducidas (iPS) del ratón establece conexiones con las células vecinas y responde a la estimulación lumínica, restaurando así la función visual en los roedores afectados por degeneración en la retina en fase terminal. Casi todas las retinas trasplantadas mostraron alguna respuesta a la estimulación lumínica. La clave del éxito fue utilizar tejido retinal diferenciado en lugar de células retinianas, usadas por la mayoría de los investigadores en este campo.

El objetivo de los investigadores japoneses es que la técnica sea próximamente probada en pacientes humanos con esta misma patología, ya que las terapias actuales tienen limitada su capacidad de detener la pérdida de visión. Para hacer que los hallazgos sean más aplicables a los pacientes, los investigadores están probando la capacidad del tejido retinal humano derivado de iPS. Mientras que la retina postrasplante puede responder a la luz después de un mes en ratones, la retina humana podría tardar entre cinco y seis meses, ya que tarda más tiempo en madurar. Si estos experimentos tienen éxito, probarán la seguridad de este protocolo evaluando cómo responde la retina del huésped al injerto.

Sin embargo, aún habrá que esperar hasta que pueda aplicarseen la clínica. «Que las células madre arraiguen y que además sean funcionales es un inicio. Lo siguiente que habría que hacer es probarlo en otros modelos de animales y si va bien lo lógico sería iniciar los clinical trials con pacientes humanos, pero este es un proceso que lleva mucho tiempo porque antes de probarlo en humanos tenemos que estar seguros de que no tiene efectos secundarios«, detalla Fidalgo.

El responsable del Grupo Stem Cells & Human Diseases del CiMus reconoce que, aunque las células madre son la clave hacia la «ansiada medicina individualizada», aún queda mucho para que puedan ser empleadas en la clínica. «En un principio, los usos de las células madre son ilimitados y en un futuro, cada paciente podría generar células madres y serían una fuente ilimitada para conseguir células para terapia celular. Sin embargo, primero hay que entender cómo funcionan para evitar su principal riesgo: la formación de tumores, pero también para que no haya rechazo inmunológico y para saber cómo coger esas células pluripotentes, diferenciarlas y convertirlas en las células que queramos utilizar en terapia celular, por ejemplo, en células productoras de insulina para tratar la diabetes», explica.

«Este fue un gran avance, porque Yamanaka consiguió lo mismo que con las células madre embrionaria, pero a partir de células diferenciadas de cualquier parte del cuerpo, lo que permitió dejar de lado la investigación con células madre, que planteaba problemas éticos y que en muchos países estaba prohibida», argumenta Fidalgo.
